Is PhilHealth identification number a requirement for COVID-19 vaccination?

MANILA, Philippines – Vaccine czar Carlito Galvez Jr. has announced during a Palace briefing on Tuesday, March 30 that potential vaccine recipients may bring any valid ID for vaccination against COVID-19.

Galvez clarified that PhilHealth identification number (PIN) is just among the “unique identifiers” and not a requirement for the inoculation as reported earlier.

“Puwede naman nilang gamitin iyong lahat ng mga puwedeng IDs po nila,” Galvez said.

Based on the Department of Health Memorandum 2021-0099, all potential vaccinee must be registered using their unique identifiers which include the PIN.

"All potential vaccine recipients shall be registered using their unique identifiers as identified during the masterlisting process such as but not limited to full name and birthday, PhilHealth identification number, system generated alphanumeric or QR or unique codes, or similar,” the memorandum states.

According to Galvez, potential vaccine recipients must undergo pre-screening and pre-listing from their respective LGUs to avoid delays and problems during inoculation. Furthermore, he encouraged potential vaccinee to get early information from their LGUs.

PhilHealth president and chief executive Dante Gierran earlier said members who do not know their PIN or non-PhilHealth members should coordinate with any PhilHealth office.

Members can verify by their PIN by contacting (02) 8441-7442 (office hours only) or by sending a text message to 0921-630-0009 with the format “PHIC callback PIN VERIF Mobile Number or Metro Manila Landline Details of your Concern.”

Meanwhile, non-PhilHealth members must accomplish a PhilHealth Member Registration Form (PMRF) which can be downloaded here. The accomplished PMRF as well as a scanned copy of a valid ID must be sent to actioncenter@philhealth.gov.ph under the subject: Register Name City/Province, Region.
